We appreciate your continued support. This is a difficult and complicated time and we appreciate you adhering to our policies.

The coronavirus disease 2019 (COVID-19) situation continues to evolve and change on almost a daily basis. Please know that we continue to receive updates from the Centers for Disease Control (CDC), and local and state health departments, in order to determine the best course of action to keep our residents safe.

If you have any specific questions or would like additional information, please reach out to our community. We will continue to keep residents, families and staff informed of any positive tests for COVID-19.

VISITATION OPTIONS

We are pleased to share that we have updated our visitation and resident outing guidelines to reflect guidelines provided by the Centers for Medicare & Medicaid Services (CMS) in conjunction with the CDC. We will continue to follow CMS, CDC, state and local health department guidelines for visitation to provide a safe environment for residents, visitors, and team members during varying phases of coronavirus disease 2019 (COVID-19). Please note the following information:

Fully vaccinated refers to a person who is ≥2 weeks following receipt of the second dose in a 2-dose series, or ≥2 weeks following receipt of one dose of a single-dose vaccine, per the CDC’s Public Health Recommendations for Vaccinated Persons.

We are allowing the following visitation options:

  • Indoor visits
  • Outdoor visits
  • Compassionate Care visits, including end-of-life visits

VISITATION GUIDELINES

  • To schedule your visits, please call 507-3588-4200 to schedule with the receptionist
  • Upon entering the community, all visitors must:
    • Check-in at the front desk and be screened upon entering the community for signs and symptoms of COVID-19 per the CDC
    • Clean your hands upon arrival and before leaving. We have hand sanitizer for your convenience and use
    • Wear a mask while entering and moving throughout the community (covering mouth and nose)
    • Social distance at least six feet between other residents and team members in the community
    • Follow instructional signage throughout the community
    • Visitors will go directly to the resident’s room or the designated visitation area
  • Residents who are fully vaccinated can choose to have close contact, including touch, with their visitor while wearing a well-fitting face mask and performing hand hygiene before and after. If both the resident and visitor are vaccinated, they may both choose to remove their masks during their visit.

VISITATION RESTRICTIONS

  • No visitors (other than compassionate care and essential caregivers) for unvaccinated residents IF the community’s county positivity rate is >10% and <70% of the residents have been fully vaccinated
  • No visitors (other than compassionate care and essential caregivers) for residents with confirmed COVID-19 until the resident has met the criteria to discontinue Transmission-Based Precautions
  • No visitors (other than compassionate care and essential caregivers) for residents in quarantine, whether vaccinated or unvaccinated, until they have met criteria for release from quarantine.
  • No visitors (other than compassionate care and essential caregivers) for residents in a unit with a current COVID-19 outbreak (one or more positive cases in a unit)
  • Outdoor visitation is preferred even when a resident and visitor are fully COVID-19 vaccinated as it generally poses a lower risk of transmission due to increased space and airflow

RESIDENT OUTING GUIDELINES

  • Quarantining is no longer needed for new admissions who are fully vaccinated or for fully vaccinated residents who have gone out for medical or non-medical appointments
    • The exception would be if the fully vaccinated resident had recent exposure to someone with suspected or confirmed COVID-19, in which case, a 14-day quarantine period should be implemented
  • Unvaccinated residents must quarantine on leaving the community, unless transported by the community’s bus to and from medical appointments

COMMUNITY UPDATES

  • CMS and CDC continue to recommend communities, residents and families adhere to the core principles of COVID-19 prevention
  • We are continuing to screen all guests and team members as they enter the community per CDC guidelines
  • Team members are required to wear appropriate personal protective equipment while at work
  • Residents are enjoying on-site group activities and restaurant-style dining
  • Transportation services available

COVID-19 VACCINATIONS

In December 2020, the CDC formally approved that the first COVID-19 vaccines go to health care workers and residents of long-term care and assisted living communities. As a result, our community partnered with CVS Pharmacy to offer and administer the COVID-19 vaccine to residents and team members. Vaccine clinics were scheduled on January 15, 2021, February 12, 2021, and March 12, 2021.

We continue to partner with local pharmacies to offer and administer the COVID-19 vaccine to new residents and team members. If family members are interested in receiving the COVID-19 vaccine, please reach out to our community.
